Day Before Oocyte Retrieval

Best Practices for the Search of Immature Cumulus-Oocyte Complexes

  • Ideal follicle size to target is 6-12mm.
  • The aspirated follicular fluid should pass through a 70-micron cell strainer. This method allows blood to pass through while trapping COCs and other cumulus cells, significantly improving the search process.
  • Then, the strainer is inverted over a dish containing search medium and briefly rinsed to allow the release of the COCs into the medium.
  • Heparinized collection media is used to reduce bloody fluid, prevent clotting, helping to improve oocyte visibility.
  • Punctures of small follicles also tend to produce thread-like blood clots. It is recommended that embryologists use needles to separate them, as COCs may be attached to them.
  • If staffing allows, it is advisable for two embryologists to perform successive searches.

Though the follicular aspirate is slightly bloodier than conventional retrievals, the combined use of the cell filter and heparinized collection media create a virtually clear fluid for search. With the proper training, embryologists are able to learn how to master this technique effectively.
